Hands-on help, or someone standing by to keep them safe. These are the six activities of daily living in the law. Tick all that apply.

3. Have they needed that help for at least 90 days?
4. Do they need substantial supervision to stay safe because of severe cognitive impairment?

For example, dementia that means someone has to watch over them so they do not wander or come to harm.

5. In the last 12 months, has a licensed practitioner certified that they are chronically ill?

A physician, registered nurse or licensed social worker, in writing.

6. Is the care given under a plan of care that practitioner prescribed?
7. Who gives the paid care?
